一、事蹟發生期限：
申請案之事蹟發生日期須介於 114年8月1日 至 115年1月31日 之間者，始得送件。

二、申請截止時間：
即日起至 115年1月26日（週一）中午 12:00 前，請將申請表及相關證明文件送至物理學系系辦公室。

Achievement Period:
The event or achievement you are applying for must have occurred between August 1, 2025, and January 31, 2026 (114/8/1–115/1/31).

Application Deadline:
The submission window is open from now until 12:00 PM on Monday, January 26, 2026.
Please submit the application form and all required supporting documents to the Department of Physics Office.
